Waiver of Liability for Minors

Important: Each participant must agree to the "Release and Waiver Liability " before participating in the Write Your Future Workshop. By completing the registration on the next page, you agree to all of the following. If you do not agree and register any way, you waive your rights and hold the Education Foundation and its partners harmless. Please read carefully before proceeding.

This release and Waiver of Liability (the "Release") is agreed to by the minor participant and his/her legal guardian in favor of the EDUCATION FOUNDATION OF SARASOTA COUNTY, a nonprofit corporation organized and existing under the laws of the State of Florida, USA and their directors, officers, employees, and agents (collectively, "Education Foundation of Sarasota County").

The Minor, desires to participate in a program sponsored by the Education Foundation of Sarasota County. I, the legal guardian, hereby freely and voluntarily, without duress, execute this Release under the following terms:

1. Waiver and Release. The guardian and minor release and forever discharges and hold harmless the Education Foundation of Sarasota County and its successors and assigns from any and all liability, claims, and demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise from the minor's participation in an Education Foundation of Sarasota County event. The guardian/participant understands and acknowledges that this Release discharges the Education Foundation of Sarasota County from any liability or claim that the guardian and minor may have against the Education Foundation of Sarasota County with respect of bodily injury, personal injury, illness, death, or property damage that may result from participation in an Education Foundation of Sarasota County event. It is also understood that the Education Foundation of Sarasota County does not assume any responsibility for or obligation to provide financial assistance or other assistance, including but not limited to medical, health or disability insurance in the event of injury, illness, death or property damage m Insurance. The guardian and minor understand that we expressively waive any such claim for compensation or liability on the part of the Education Foundation of Sarasota County beyond what may be offered freely by the representative of the Education Foundation of Sarasota County in the event of such injury or medical expense.
2. Medical Treatment. The guardian md minor hereby release and forever discharge the Education Foundation of Sarasota County from any claim whatsoever which arises or may hereafter arise on account of any first-aid treatment or other medical services rendered in connection with an emergency during the minor’s time with the Education Foundation of Sarasota County.
3. Assumption of Risk. The guardian understands that the minor's time with the Education Foundation of Sarasota County may include activities that may be hazardous to them including, but not limited to, local transportation to and from sites. As the guardian for the said minor I hereby expressly assume the risk of injury or harm in these activities and release the Education Foundation of Sarasota County from all liability for injury, illness, death or property damage resulting from the activities of the minor's time at the Education Foundation of Sarasota County.
4. Photographic Release. As the guardian of said minor I grant and convey unto the Education Foundation of Sarasota County all right, title, and interest in all photographic images and video or audio recordings made by the Education Foundation of Sarasota County during the minor's time with the Education Foundation of Sarasota County.
5. Content. The guardian understands that the minor may be exposed to discussions about sensitive topics and that the participant must keep this information confidential.
6. Registration fee: I understand that the registration fee is due prior to the workshop and that this fee is nonrefundable.
7. Attendance: Students must attend all workshop sessions. Workshops are for students only; no parents, siblings or friends may accompany the participant.
8. Other. As the participant expressly agree that this Release is intended to be as broad and inclusive as permitted by the laws of the State of Florida in the United States of America, and that this Release shall be governed by and interpreted in accordance with the laws of the State of Florida. I agree that in the event that any clause or provision of this Release shall be held to be invalid by any court of competent jurisdiction, the invalidity of such clause or provision shall not otherwise affect the remaining provisions of this Release which shall continue to be enforceable.
